Comparative Analysis of Postoperative Complications of the Axillary Osmidrosis According to The Surgical Method

Abstract
Purpose: In axillary osmidrosis, one is often embarrassed with unpleasant odor from bacterial
decomposition of his apocrine sweat gland secretion. Many surgical excision techniques has been
developed to cure this condition but also with numerous complications. Recently, aspiration techniques
has been adopted and the comparative analysis with conventional techniques is needed.

Materials and Methods: From 2005 to 2009, twenty four osmidorosis patients(15 males and 9 females)
underwent 8 surgical subdermal excisions and 16 aspirations. Follow-up(at least 10 months, average 27.3
months) could be done on 7 of 8 surgical subdermal excision patients(87.5%) and 10 of 16 aspiration
patients(62.5%) with chart review and questionnaires through telephone interview. Other patients were
dropped out due to rejection or address change. Effectiveness & satisfaction for each technique was rated
in four grades as very good, good, poor, very poor and very satisfactory, satisfactory, dissatisfactory,
very dissatisfactory, in decreasing order respectively.

Results: In effectiveness item, all surgical excision group was rated as very good(4/7, 57.1%) and
good(3/7, 42.9%) in contrast to very good 1(10%), good 5(50%) and poor 4(40%) in aspiration group.
For overall satisfaction criteria, 5 of 7(71.4%) surgical excision group and 4 of 10(40%) aspiration group
patients reported as satisfactory and very satisfactory. As complication, 1 case each of marginal necrosis
and resultant hyperpigmentation, wound dehisence, and hematoma occurred in surgical excision group(3/7,
42.9%) without any complication in aspiration group(0%).

Conclusion: In summary, surgical excision group was superior than aspiration group in effectiveness and
long-term satisfaction but with the expense of short-term complications. Both techniques should be
applied individually according to the condition of the patients.
